Do you see CQCTrayMon.exe in the start menu? Typically it'll be in the 'extra icons' popup, so you have to hit the little up arrow to see the other icons.

CQCTrayMon.exe is not in the startup folder...but Task Manager shows that it is running (and the drivers are clearly connected to it). Note that I did not manually launch it so the auto-start on login appears to be working. I did check the extra icons popup window and it is not there. Also clicked the "customize" link to confirm that it isn't set to hide...nowhere to be found.

Not sure what to tell you there, dude. If it's running, then clearly it was allowed to start up as a tray app, so I don't know why it wouldn't be visible.
It's not started via the Startup folder, it's started up via the Windows task manager, since that's the only really good way to do it if it needs to start up with Admin rights.
