Wired control of a TV

I dont know why I was think of a hospital, but I was and it dawned on me.

All the TV's there have an add-on of a wired remote at the bed. THis stops the IR remote going walkabout. So from this, does any one know what is done to get this functionality and how simple would it be to add to a TV.

This may be a good option to add to an older TV that is out of warranty.

I've never seen an HDTV in a hospital though.
